CHEER UP for DOWN SYNDROME

The purpose of the CHEER UP for DOWN SYNDROME Care Card Program is to offer assistance for Down Syndrome Alabama families whose loved one with Down syndrome is experiencing long-term hospitalizations for complex surgeries, medically fragile conditions, and extended illness recovery periods.

1. 1. 1. Applicants must be a current member of Down Syndrome Alabama. 
      2. Applicants must currently be in-patient hospitalized.
      3. Applicants must have been in-patient hospitalization for forty-eight hours.
      4. Applicants may only apply one-time within between January 1-December 31, of any given year.
      5. Applicants must apply online through the website, or someone may apply for them, and provide the necessary information.
      6. Upon approval, applicants will receive a $25.00 gift card mailed to indicated address.  

*The form will be reviewed, membership will be verified, and application will be approved by a committee. Due to limited funding, this program will only last until current grant funding runs out, and therefore isn’t a guarantee.  Likewise, being a pilot program, upon initial data collection from pilot year, we reserve the right to make adjustments to the program accordingly.
